You can submit the Application by going to our CONTACT page and attaching the competed documents to the provided form. Alternatively, you could attach the completed form to an email and send it to:

Clicking on the appropriate link will bring you to where you can look at the document needed. Select the add text box in the top right-hand side of the screen. Using this tool, you can fill out the required information. After completion click on the save as button which will prompt you to save it to your device. After saving you can then attach it as a file to the email above or through the message system on our CONTACT page.
Alternatively, you can simply print the provided resources out by following the first step of the last paragraph. After printing out the page simply fill it out and mail it to the provided mailing address on our contact page. (Fax also works)
Adults who are interested in the School of Practical Nursing Program must make the application through the SPN Coordinator. Applicants must pass an entrance exam and be interviewed by the faculty or a representative before being considered for admission.
The School of Practical Nursing applicants MUST hold a high school diploma or TASC/GED upon submission of the application.
1.) Pass the scheduled Pre-entrance Exam. There is a non-refundable fee for this exam.
2.) Complete the FIT School of Practical Nursing application
3.) Provide an official grade transcript or high school equivalency exam
4.) Complete an interview
5.) Provide two references
6.) A non-refundable seating fee of $250 is required upon acceptance into the program.
7.) This fee will later be applied toward tuition costs upon acceptance and enrollment into the LPN program.
We enroll approximately twenty students each year.
Classes start in March
Upon acceptance be sure to fill out the Release Form, as well as download the Student Handbook.
TEAS
The TEAS test has a $75 fee.

Testing may be paid for by check, cash, or money order.
You must bring a valid photo ID (such as a driver’s license) and a mask to your test appointment.
Testing for the class of 2023 will begin in the fall of 2022
TEAS Prep
TEAS test prep is available at no charge through our Adult Education Department.

Request Transcripts
To request transcripts there is a $5 fee.
We DO NOT accept cards.
Please send the full amount through our mailing address or at our physical location.

Financial Aid is available to qualified students accepted into the program. Federal loans are NOT available.
Financial aid is available to qualifying adult students in the form of Federal Pell Grants, Workforce Investment Act, WV Department of Vocational Rehabilitation, Veterans Affairs, Trade Readjustment Allowance and UMWA Career Center.
